1. Acceptance of Terms
By creating an account or using Age Well Motion ("the Service"), provided by Robert Fausel, PT ("we", "us"), you agree to these Terms of Service. If you do not agree, do not use the Service.
2. Telehealth Services
The Service provides virtual longevity mobility assessments, personalized exercise plans, and clinician-reviewed movement guidance via video and digital tools. Care is provided by Robert Fausel, a licensed Physical Therapist.
Telehealth has limitations. The provider may determine that an in-person evaluation is needed. We do not provide emergency care. If you are experiencing a medical emergency, call 911.
3. Eligibility & Accounts
You must be 18 years or older and able to provide informed consent. You agree to provide accurate health information and to keep your account credentials confidential. You are responsible for activity on your account.
4. Payment & Refunds
The virtual assessment is $79, charged at booking via Stripe. If you cancel at least 24 hours before your appointment, you may request a full refund. Cancellations within 24 hours and no-shows are non-refundable. Released exercise plans are non-refundable once delivered.
5. User Responsibilities
- Complete the safety pre-screen honestly.
- Stop any exercise that causes pain, dizziness, or distress and contact your physician.
- Do not share your account or recorded sessions.
- Comply with all applicable laws.
6. Intellectual Property
All content, plans, and materials provided through the Service are owned by Robert Fausel, PT and licensed to you for personal, non-commercial use only.
7. Disclaimers
The Service is provided "as is" without warranties of any kind. We do not guarantee specific health outcomes. Educational content is not a substitute for emergency medical care or advice from your primary physician.
8. Limitation of Liability
To the maximum extent permitted by law, our total liability for any claim related to the Service is limited to the amount you paid us in the 12 months prior to the claim.
9. Termination
You may close your account at any time. We may suspend or terminate accounts that violate these Terms or pose a clinical safety concern.
10. Changes
We may update these Terms. Material changes will be communicated via email or in-app notice. Continued use after changes constitutes acceptance.
