Age Well Motion ("we", "us") is a telehealth mobility service provided by Robert Fausel, PT. This policy explains how we collect, use, store, and protect your information, including Protected Health Information ("PHI").

- Account info: name, date of birth, phone, email.
- Health info: longevity profile, safety screening answers, mobility goals, clinical notes, exercise progress.
- Movement videos: short clips you upload for clinical review.
- Payment info: handled by Stripe; we never store full card numbers.
- Technical info: device type, browser, IP, session logs for security and debugging.
3. How We Use Your Information
- Provide telehealth assessments and personalized exercise plans.
- Allow Robert Fausel, PT to review your screening, videos, and progress.
- Send appointment confirmations, reminders, and plan updates.
- Maintain audit logs of clinical actions for safety and compliance.
- Improve the Service and prevent fraud or abuse.
We do not sell your personal or health information.
4. HIPAA & Safeguards
We treat your health information under HIPAA-aware practices, including:
- Encryption in transit (TLS) and at rest.
- Row-level security so only you and your clinician can access your records.
- Access controls and audit logging of clinical actions.
- Business Associate Agreements (BAAs) with infrastructure providers handling PHI.
5. Sharing
We share information only with:
- Robert Fausel, PT and authorized clinical staff.
- Service providers (hosting, database, payments, video, email) under written BAAs or data-processing agreements.
- Legal authorities when required by law.
- Anyone you direct us to (e.g., your physician), with your written consent.
6. Your Rights
- Access a copy of your records.
- Request corrections to inaccurate information.
- Request deletion of your account and associated data, subject to legal retention requirements.
- Withdraw consents (telehealth, video storage, data use) at any time in your account settings.
- File a complaint with us or the U.S. Department of Health and Human Services.
7. Data Retention
We retain clinical records for the period required by applicable state and federal law (typically 7+ years for adult records). Account info is retained while your account is active and deleted on request unless retention is legally required.
8. Children
The Service is for adults 18+. We do not knowingly collect information from children.
9. Changes
We will notify you of material changes by email or in-app notice. The "Last updated" date at the top reflects the current version.
